1. Chose the card design that you would like to order, either click on one of the order links shown next to the card on the designs page, or select the card from the drop down menu on the ordering page.

2. The disclaimer screen will next be shown, read the points and then either decline or accept them. You must accept the disclaimer to order the card.

3. After accepting the disclaimer the order form page for the card that you selected will be shown. Your personal order number is shown at the top, you will need to make a note of this because you will need it in case you need to contact us about your order.

4. Fill in the spaces on the order form by typing into the boxes. If you were referred by a reseller type the resellers number into the space at the top, if not leave it blank.

5. Print off the order form.

6. Attach your photo to the relevant space on the order form (blue tack, paper clip, sticky back plastic etc.).

7. Sign the signature that you want printed on the card inside the signature box on the order form. For best results use a think pen with blue/black ink. (Note, not all cards have signatures printed on them, and so not all order forms have signature boxes.)

8. You also have to sign the disclaimer at the bottom of the order form, this must be your real signature.

9. Send off your order form to the address shown at the top of the order form (ID-Shop.co.uk, P.O. Box 752, Stockton, TS19 1BD), also enclose payment. This can be cash, postal orders or cheques (payable to "P.C.S.") for the price of the card, either £10 (€20, AU$35) £15 (€30, AU$45).

If for any reason you are unable to print the order form you can either copy out the whole form by hand, or contact us and we will send you out a mail order catalogue.
